In this session we will discuss recommendations, guidance and guidelines relating to personalised care planning and patient care for adult and paediatric patients. We will also consider when a second opinion or referral may be indicated.
- · Apply clinical guidelines to Personalised Care Planning · Identify indications for onward referral or second opinion · Discuss shared decision making in relation to formulating personalised care plans
Aims
Understand and integrate relevant clinical guidelines into the development of personalised care plans. Demonstrate the ability to tailor care plans to individual patient needs while adhering to established clinical protocols. Recognise clinical scenarios that necessitate referral to specialists or seeking a second opinion.
